APRT(T) – Declaration of Interest

To identify your interest to participate in the APRT(T) certification process, please complete the form below. Your application will be reviewed by CAMRT and delegates from the APRT Certification Validation Committee within approximately 4 weeks. As you wait for a response, we encourage you to review the APRT(T) certification handbook.

  • If you are deemed eligible, CAMRT will contact you in writing to formally register for the first phase of the APRT(T) certification process – the portfolio submission.
  • If you are not deemed eligible, CAMRT will contact you in writing to provide feedback on your Declaration of Interest form and associated submitted documents. Should an opportunity arise for you to provide additional information and resubmit, this will be included in your feedback information.

Eligibility

If yes, you are required to submit proof of this (e.g., letter from employer(s), CV with employment dates).
Please provide evidence of academic education completed to date (e.g., transcript, certified degree) or proof of equivalence as required within the eligibility criteria.

Professional Experience

In addition to proof of clinical practice and education (or equivalent) to meet the eligibility criteria above, please submit a copy of your CV, which should include publication, presentation, grants, awards and other relevant professional information.

By submitting your Declaration of Interest, you are acknowledging and consenting to the following:  The CAMRT retains the right to request additional documentation from an applicant (e.g., letters of reference) in order to complete the assessment of eligibility. All documents and information obtained by CAMRT during the certification process become the property of CAMRT and will remain confidential. Applicants are therefore requested to provide scanned copies and should not mail original documents. All personal information is managed in accordance with the CAMRT Privacy Policy. The CAMRT has permission to use your personal information and any information submitted as part of the APRT(T) certification process (from Declaration to Credentialization) to support the determination of your application. You are also acknowledging that you will have reviewed the APRT(T) certification handbook and, specially, will uphold tenants stated therein, including but not limited to maintaining professional conduct and confidentiality within and beyond the certification process.
